We Focus on What Matters Most
As a nation, more than 75% of our health care spending is for people with chronic conditions, according to the National Center for Chronic Disease Prevention and Health Promotion (part of the Centers for Disease Control and Prevention).
SMGF provides free wellness screenings for children and adults, with a focus on detecting asthma and diabetes. Asthma was recently cited as the leading cause of hospitalization for New Jersey children.* Diabetes has become a top 10 leading cause of death in New Jersey (and the United States) in the past decade. By focusing on these two chronic illnesses, SMGF hopes to help make a positive impact on the community.
With your help, we can promote healthy living and targeted education programs like these and continue to build a strong foundation for healthy communities.
